A puffy under-eye with a hollow shadow below is often caused not by too much fat but by fat pushing forward while the groove beneath is empty. Simply removing the fat can make the groove sink further.

Under-eye fat repositioning approaches from inside the conjunctiva and moves the bulging fat into the hollow groove. With no skin incision there is no visible incision line, and no stitches to remove. If the skin is loose, lower-lid skin excision is considered as well.

It is done under sedation, and the time for swelling to subside and follow-up visits are explained in consultation.

Finding the causeToo much fat, or fat pushed forward?
The bulge is often due less to excess fat than to fat that has pushed forward, while the groove beneath it is empty. Simply removing the fat may make the groove sink further.

RepositioningApproached from inside the conjunctiva
Fat that has pushed forward is moved and spread into the hollow groove. As the unevenness lessens, shadows look lighter too. No skin incision is made.

Fixation to the periosteumReduces the chance of bulging again
The repositioned fat is fixed to the periosteum beneath the tear trough. If there is also skin laxity, Lower Blepharoplasty is considered as well.

Frequently Asked Questions

Isn't it better to just remove under-eye fat?

Removing fat can make the area look hollow with age. Repositioning is the default, with partial removal added depending on your condition.

Will a scar remain?

No incision line shows on the skin, because we approach from inside the conjunctiva. If the skin also sags, lower blepharoplasty is considered.

Can I have it if the area is lumpy after filler or fat grafting?

Yes. An uneven under-eye surface after fat grafting or filler can also be discussed. We check the condition of the remaining filler or fat before deciding on the method.

How long is recovery?

How long swelling takes to go down differs from person to person. If you have an important event coming up, please tell us at your consultation and we will plan around it.
