Contracted nose revision is not simply lengthening a nose that has become short.

A long-standing implant or repeated inflammation often leaves the skin and soft tissue contracted and hardened, causing a short-nose deformity in which the tip turns up, the alae lift with it, or the columella looks short.

Bonita's contracted nose revision does not force contracted tissue to stretch. It assesses how hardened the skin and soft tissue are and how much tissue is missing, aiming to rebuild a nose line that stays stable.

For You

If these concerns sound familiar

  • If the tip turns up and the nose looks short
  • If the alae lift too and the nostrils look prominent
  • If the skin over the implant has become hard
  • If your nose has changed shape after repeated nose surgery or inflammation
  • If you want a thorough consultation and timing assessment before reconstruction

Hardened tissue first, slowly

Why

Why is contracted nose revision not simple lengthening?

01

The skin and soft tissue themselves have hardened

Simply lowering or lengthening the tip can make the already hardened tissue contract again and bring back the short-nose deformity. The hardened tissue has to be released first.

02

Tissue volume that has been lost needs to be replenished

Repeated surgery and inflammation often reduce the skin and soft tissue themselves. The missing tissue must be replenished with autologous grafts such as rib cartilage, or in some cases autologous skin grafts.

03

Timing and staging of surgery matter

A contracted nose should be rebuilt only after waiting until the skin is at its softest (a set period after implant removal). Depending on the condition, staged surgery rather than a single operation may be needed.

Process

How contracted nose revision proceeds

01

Analysis of anatomical causes and proportions

Design begins with accurate analysis

  • Check of how far skin and soft tissue have hardened (contracted)
  • Analysis of short-nose deformity (tip lift, columella and alar retraction)
  • Assessment of remaining cartilage volume and tissue adhesions
  • Review of inflammation/infection history and timing of revision
  • Tailored reconstruction plan
02

Tailored correction centred on cartilage and structure

Releasing and replenishing matter more than stretching

  • Releasing adhered and contracted tissue
  • Rebuilding bridge and tip support with autologous tissue grafts such as rib cartilage
  • Replenishing missing skin with autologous skin grafts where needed
  • Staged surgical planning (first and second stages separated where needed)
03

Tissue stabilisation and stage-by-stage recovery care

Results are completed through a stable recovery

  • Stabilisation care for the released tissue
  • Swelling and recovery care
  • Follow-up to prevent re-contracture
  • Long-term line maintenance

Recovery

  1. Right after treatment

There is swelling and bruising; the nasal splint stays on and cold compresses are used.

  2. During recovery

The splint and stitches are removed. While the main swelling subsides, avoid pressure on the nose such as from glasses.

  3. As it settles

As residual swelling in the tip subsides, the final shape appears.

Recovery time and follow-up visits vary with the extent of treatment and your individual condition.

FAQ

Frequently Asked Questions

When can contracted nose revision be done?
We reconstruct once the skin has softened enough. Depending on the condition, the surgery may be done in stages.
Is it enough just to take the implant out?
If only the implant is taken out and the surrounding capsule is left, the tissue can contract again or the nose can look unnatural. We decide on capsule removal and post-removal support together.
Where will the scar be?
For the nose, scars are mostly inside the nostrils or on the columella, and we manage them so they fade over time.

※ Bleeding, infection, inflammation and other side effects may occur after surgery or treatment. Please decide after a full consultation with your doctor.
